Job summary

Responsibilities include delivering individual, family, couples and group therapies, coordinating care among primary care, social work, psychiatry, and nursing, and maintaining HIPAA-compliant documentation while using community resources for referrals.

Qualifications

• Active state licensure as LCSW or LPC.
• Master’s Degree in Social Work with clinical coursework and field experience, or Master’s Degree in Counseling with clinical coursework and field experience.
• BLS – Basic Life Support.

Responsibilities

• Conduct comprehensive clinical assessments when appropriate to level of care.
• Plan treatment based on client needs and resources, coordinating with clinic and community resources.
• Provide individual, family, couples and group therapy.
• Coordinate multidisciplinary team involvement to ensure continuity of services.
• Maintain HIPAA-compliant documentation and timely data entry.
• Offer crisis assessment and intervention as needed.
• Refer and consult on community resources and patient education materials.
