Job summary

Qualifications

• Master’s degree in social work required.
• Experience in health care or integrated primary care or social service setting preferred.
• Licensed Specialist Clinical Social Worker (LSCSW) license or eligibility required.

Responsibilities

• Perform psychosocial assessments of patient/family situations.
• Coordinate referrals with care managers to ensure timely services.
• Provide short-term counseling and crisis intervention as needed.
• Utilize community resources to support patient needs and social determinants.
• Document in EMR and communicate with care team for continuity of care.
• Advocate for patient rights and self-determination while ensuring safety.
