Job summary

Upham's Community Care seeks an Integrated Behavioral Health Clinician to deliver therapy using CBT, DBT, MI and ACT, and coordinate wrap-around services. You will perform intake, treatment planning, and outreach within the Behavioral Health department, aligning with the UCC Medical Home model.

The role requires a MSW and MA licensure, with at least 2 years in community settings and strong collaboration across teams. Evening availability and EMR documentation are expected.

Qualifications

• Master's in Social Work and current licensure in MA required (LCSW/LMHC).
• Minimum 2 years of community-based behavioral health experience.
• Experience with trauma-informed care and interdisciplinary teams.

Responsibilities

• Conduct intake screening, assessments, and triage for levels of care.
• Provide individual, family, and group behavioral health treatment.
• Document in the electronic medical record and manage risk/crisis scenarios.
• Collaborate with other departments and external agencies as needed.
