Job summary

The Northeast Treatment Center in Philadelphia is seeking a Targeted Case Manager. The role supports adults with serious mental illness and substance use disorders by linking to behavioral health services, coordinating resources, and guiding treatment plans to help individuals obtain independence.

Responsibilities include arranging evaluations, connecting to medical and community services, and advocating for participants.

Qualifications

• Bachelor's degree in mental health or related human services field with 2 years’ experience in Targeted Case Management in Philadelphia, OR a Master’s degree in human services, psychology or Social Work.
• Experience serving adults with serious mental health and substance use disorders.
• Valid Driver’s License with safe driving record and proof of insurance.
• Experience navigating multi-system services and knowledge of community resources.
• Respect for adults and families; understanding of cultural competence and recovery.
• Adult and Pediatric CPR Certification.

Responsibilities

• Coordinate meetings and evaluations for participants.
• Coordinate access to medical and treatment services as needed.
• Outreach to participants and families for service coordination.
• Assist participants and families in accessing community resources.
• Advocate for participants’ needs.
• Ensure continued communication and integration among all service providers.
• Monitor implementation of clinical strategies with the team.
