Job summary

VILLAGE-VIRGIN ISLANDS PARTNERS IN RECOVERY, INC is seeking a Behavioral Health Case Manager in Christiansted, VI to provide wrap-around services and coordinate care for clients with substance use and mental health needs.

Responsibilities include case management, HMIS data entry, and collaboration with a treatment team to develop individualized plans and linkage to resources, housing, and public entitlements as needed.

Qualifications

• Minimum high school diploma; bachelor’s preferred in social services.
• Ability to understand substance use/mental health concepts.
• Familiarity with community health resources.
• Valid VI driver license.

Responsibilities

• Maintain and understand the Behavioral Healthcare Home model and treatment principles.
• Participate as a member of the treatment team and assist with case management.
• Ensure HMIS data is collected and entered in a timely manner.
• Develop client case management plans with input from the client.
