Qualifications

Responsibilities

  • Conduct case management assessments to identify clients' behavioral health, recovery, social, and community-support needs.
  • Develop and maintain individualized case management plans in collaboration with clients and the clinical treatment team.
  • Provide ongoing case management services and monitor client progress toward identified goals.
  • Coordinate services with internal and external providers to ensure comprehensive and integrated care.
  • Assist clients with identifying and accessing appropriate community resources and support services.
  • Help clients address barriers that may interfere with treatment or recovery.
  • Advocate for clients and help them navigate behavioral health, substance use treatment, healthcare, and community-service systems.
  • Maintain communication with outside providers, referral sources, family members, and recovery supports.
  • Participate in treatment team meetings and communicate needs, progress, and discharge considerations.
  • Assist with discharge planning, aftercare coordination, referrals, and continuity-of-care arrangements.
  • Follow up on referrals and outside appointments to ensure access to services.
  • Maintain accurate documentation in the electronic health record in accordance with regulations and Aviv Recovery policies.
  • Maintain client confidentiality and comply with HIPAA and 42 CFR Part 2.
  • Attend supervision, staff meetings, trainings, and professional development activities.
  • Collaborate with counselors, clinical leadership, medical providers, admissions staff, and other Aviv Recovery team members.
  • Perform other duties as assigned by Clinical Director or leadership team.

Job description

Aviv Recovery is seeking a Behavioral Health Case Manager to support clients receiving substance use disorder and behavioral health treatment services. The Case Manager will work closely with clients, clinical staff, referral partners, and community providers to help coordinate services, address barriers to treatment, and support each client's ongoing recovery and discharge planning needs.

The Behavioral Health Case Manager will provide case management services in accordance with applicable Virginia Administrative Code requirements, agency policies and procedures, and Aviv Recovery's standards of care.

Key Responsibilities
  • Conduct case management assessments to identify clients' behavioral health, recovery, social, and community-support needs.
  • Develop and maintain individualized case management plans in collaboration with clients and members of the clinical treatment team.
  • Provide ongoing case management services and monitor client progress toward identified goals.
  • Coordinate services with internal and external providers to help ensure clients receive comprehensive and integrated care.
  • Assist clients with identifying and accessing appropriate community resources and support services.
  • Help clients address barriers that may interfere with treatment or recovery, including transportation, housing, employment, medical care, benefits, legal concerns, and other social-service needs when applicable.
  • Advocate for clients and assist them in navigating behavioral health, substance use treatment, healthcare, and community-service systems.
  • Maintain communication with outside providers, referral sources, family members, recovery supports, and other authorized individuals as clinically appropriate.
  • Participate in treatment team meetings and communicate relevant case management needs, barriers, progress, and discharge considerations to the clinical team.
  • Assist with discharge planning, aftercare coordination, referrals, and continuity-of-care arrangements.
  • Follow up on referrals and outside appointments to help ensure recommended services are accessed.
  • Maintain accurate, complete, and timely documentation in the electronic health record in accordance with Virginia regulations, payer requirements, accreditation standards, and Aviv Recovery policies and procedures.
  • Maintain client confidentiality and comply with HIPAA, 42 CFR Part 2, and other applicable confidentiality requirements.
  • Attend required supervision, staff meetings, trainings, and professional-development activities.
  • Collaborate professionally with counselors, clinical leadership, medical providers, admissions staff, peer support staff, and other members of the Aviv Recovery team.
  • Perform other duties as assigned by the Clinical Director or leadership team.
Qualifications
  • Experience working with individuals with substance use disorders and/or co-occurring mental health needs preferred.
  • Knowledge of community behavioral health and recovery resources preferred.
Minimum Required Skills and Experience
  • Minimum of two years of behavioral health, substance use disorder, human services, or case management experience preferred.
  • Knowledge of applicable Virginia behavioral health regulations and documentation requirements.
  • Strong communication and interpersonal skills.
  • Strong organizational and time-management skills.
  • Ability to maintain accurate and timely clinical documentation.
  • Ability to manage multiple clients, priorities, and deadlines simultaneously.
  • Ability to work independently while also functioning effectively as part of an interdisciplinary treatment team.
  • Ability to establish appropriate professional boundaries and maintain client confidentiality.
  • Strong problem-solving and resource-coordination skills.
  • Experience using electronic health records preferred.
Preferred Experience
  • Previous experience working in a substance use disorder treatment setting.
  • Experience working in outpatient, Intensive Outpatient Program (IOP), Partial Hospitalization Program (PHP), residential, or community behavioral health settings.
  • Familiarity with community resources and behavioral health providers throughout Hampton Roads.
  • Experience coordinating transitions between different levels of care.
