RVNAhealth is seeking an experienced Intake Triage RN to join our dedicated team and serve as the vital link between our community, referral sources, and care teams across all service lines-with a focus on hospice services.

In this critical front-line role, you'll be the first clinical voice in the patient care journey, using your expertise to evaluate referrals, engage with patients and families about care options, and ensure a smooth and timely transition into services. If you thrive in a dynamic, fast-paced environment where clinical knowledge meets meaningful communication, we'd love to meet you.

This is a full-time position, Monday-Friday, based in the Ridgefield, CT Office. (May include occasional weekend/holiday coverage.)

What You'll Do:

  • Coordinate and review referrals for care across all service lines, with a primary focus on hospice admissions.
  • Evaluate hospice eligibility and lead compassionate goals-of-care conversations with patients and families.
  • Communicate closely with physicians, clinical staff, and referral sources to ensure smooth care transitions and outstanding service.
  • Assign acuity levels and initiate appropriate documentation, including coding and obtaining verbal orders.
  • Ensure all referrals meet clinical and regulatory standards and are processed efficiently.
  • Participate in team meetings and identify opportunities for process improvements.
  • Represent RVNAhealth with professionalism and empathy in all interactions.

What You Bring:

  • Current RN license in the State of Connecticut (Required).
  • Strong clinical judgment and experience in hospice, home health, or community care.
  • Working knowledge of diagnosis coding and care coverage criteria.
  • Proven ability to manage multiple priorities calmly and effectively.
  • A natural talent for building trust and guiding families through sensitive decisions.
  • Tech-savvy and comfortable using clinical software for documentation and workflow.
  • Knowledge of healthcare sales, business development, or referral growth is a plus.

Compensation:
Th pay range listed represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
