Job summary

This full-time role involves screening, assessment, and referrals to best-match services for adults with mental health needs. Required qualifications include MSW with LMSW/LLMSW licensure, strong clinical assessment skills, DSM-V knowledge, crisis intervention experience, and teamwork within a multidisciplinary setting.

Qualifications

• Strong clinical assessment and writing skills.
• Knowledge of DSM-V and diagnostic formulation.
• Ability to diagnose in assessment and treatment planning.
• Experience with crisis intervention and safety planning.
• Familiarity with community resources and referrals.

Responsibilities

• Perform centralized intake screening for assigned programs.
• Evaluate level of care and determine appropriate service referrals.
• Document assessments and treatment planning notes per policy.
• Provide crisis intervention and coordinate with multidisciplinary team.
