INTAKE SPECIALIST - Delaware County

CareStar, Inc. is seeking an Intake Specialist in the Clinical Department to conduct program assessments using established tools and processes. The Intake Specialist will provide eligibility recommendations according to program and CareStar guidelines.

THE POSITION

CareStar is looking for motivated individuals who:

  1. Complete visits to assess eligibility for specific programs and conduct safety assessments, in compliance with CareStar and department guidelines.
  2. Document all applicant and case-related contacts and interventions in the applicant's record per agency and CareStar policy, including standards of content and timeliness.
  3. Process Program Eligibility Assessment Tool (PEAT) or other tools and all related documents per agency and CareStar policy.
  4. Provide assistance with community resource referrals as appropriate.
  5. Identify and report high-risk situations as required per agency and CareStar policy.
  6. Hold an active RN license.
  7. Have 12 months of experience in home and community-based services within the past 5 years.
  8. Knowledge of Medicare, Medicaid, Home Health Care, and Ohio Home Care waiver programs is desirable.
  9. Adhere to the CareStar Rules in performing job responsibilities.
  10. Understand and comply with CareStar Policies and Procedures.
  11. Maintain confidentiality regarding patient information, with any unlawful disclosures treated as serious disciplinary offenses.
  12. Follow the Acceptable Use Policy when using CareStar’s information systems.

This position is based in Delaware County and covers surrounding counties.
