JOB PURPOSE:

Conduct initial eligibility screening for the Family and Youth Services Support Program to ensure individuals in need of crisis pregnancy services, foster care services, and community services are connected to appropriate community resources. This program serves 9 Texas counties: Austin, Brazoria, Chambers, Fort Bend, Galveston, Harris, Liberty, Montgomery, and Waller. The role supports rural regions and involves travel. This is a grant-funded position.

Qualifications

ESSENTIAL JOB RESPONSIBILITIES:

  • Develop effective, trusting relationships with participants using trauma-informed care and a strength-based approach. Demonstrate positive interactions, verbal guidance, and effective communication.
  • Conduct initial eligibility screening, advocate for eligible participants, provide community referrals, complete intake evaluations, and set up initial client files for those assigned to a Service Coordinator.
  • Answer calls and emails, assist with office duties.
  • Document case notes daily, maintain participant files, prepare reports, and enter data into Salesforce. Complete incident reports as needed.
  • Communicate with multidisciplinary teams to assist participants in obtaining and maintaining services.
  • Perform other duties as assigned.
  • Provide exceptional customer service, prioritizing the child, family, Veteran, or client to support our mission to empower individuals and communities.

ESSENTIAL QUALIFICATIONS:

Education: Bachelor's degree preferred

Experience: 2+ years in a social service agency and customer service environment; experience with mental health or homeless populations is a plus.

Attendance: Must maintain regular, acceptable attendance.

Licenses: Valid driver's license with a clear record.

Vehicle: Daily use of a vehicle required, with notice.

Other: Bilingual in Spanish/English preferred. Willing to travel as needed.
