Job summary

Integrated Home Care Services (IHCS) is seeking an Intake Specialist to join our team in New York. You will handle referrals from hospitals, SNFs, PCPs, and specialists, ensuring accurate intake and insurance verification for home-based nursing, equipment, and therapies.

You will work in a collaborative, patient-focused environment, using EMR/EHR systems, communicating clearly with providers and patients, and prioritizing tasks to support seamless care.

Qualifications

• Effective verbal and written communication skills.
• 1 year of customer service experience.
• Ability to use computer programs and EMR/EHR systems.
• Ability to self-motivate and work independently.
• Ability to multi-task, set priorities and manage time effectively.
• Bilingual Spanish is a plus.
• Certification a plus.

Responsibilities

• Process incoming referrals from hospitals, skilled nursing facilities, PCPs and specialists.
• Verify insurance eligibility for services including nursing, medications, and equipment for home care.
• Collaborate with clinical team to ensure accurate and timely intake.

What Will You Be Doing

The Intake Specialist is responsible for processing incoming referrals from hospitals, skilled nursing facilities, primary care providers (PCPs), and specialists. This position ensures the verification of insurance eligibility for services including nursing, medication, and equipment needed for patients in the home setting.

What Will You Need To Succeed

• Effective verbal and written communication skill
• 1 Year of Customer Service Experience
• Ability to use various computer programs and applications, IE EMR/EHR
• Ability to self-motivate and work independently
