Forest View Hospital in Grand Rapids, MI is seeking a Psychiatric Triage/Intake Registered Nurse (RN) to join our private 108-bed behavioral health facility. You will interview patients, review medical history and medications, and coordinate care with ARC staff and physicians to ensure a seamless transition to the unit.

Night, day, and PRN shifts are available across multiple roles. Michigan RN license and BLS/CPR certification required; prior psychiatric experience preferred.

Qualifications

• Current license to practice nursing in Michigan or eligible to receive/renew.
• Previous psychiatric nursing or assessment experience.
• Current CPR or BLS certification.

Responsibilities

• Demonstrates professionalism and empathy with patients, families and staff throughout admissions.
• Provides comfort and support to ease anxiety during intake and admissions.
• Documents medical history and psychiatric status accurately as part of assessment and admissions.
• Assists patient with transfer to the unit and supports smooth transition.
• Maintains knowledge of hospital policies and legal aspects affecting nursing practice.
• Handles crisis calls, makes referrals and completes assessments.
• Performs other duties as assigned.

As a part of our continued success and growth, we are seeking qualified applicants for a Psychiatric Triage/Intake Registered Nurse (RN).

The Psychiatric Triage/Intake RN works closely with other Admissions and Referral Center (ARC) staff, the physicians, and the patient and their families to establish trust and rapport when interviewing a patient’s medical history, current medications, current health updates, vitals, and the nursing processes to both adult and adolescents to ensure the patients experiences a seamless transition to the unit.

Essential Job Responsibilities

• Demonstrates a high level of mental and emotional tolerance sensitivity, diplomacy, sound judgment, and a professional attitude when relating with patients, families and co-workers throughout the admissions process.
• Provides support and concern for the patient to ease anxiety during the intake and admissions process.
• Responsible for documentation of patients’ medical history, and findings as part of the assessment and admissions process, continually assesses and reassesses the patient’s medical and psychiatric status and changing needs.
• Accompanies the patient to the unit and assists with the transition to the unit.
• Performs duties with frequent interruptions, in close quarters and under time constraints in a professional manner.
• Maintains a current knowledge of the legal aspects of nursing and of the policies, procedures and regulations of the hospital.
• Handle incoming crisis calls, make referrals and complete assessments
• Performs other duties as assigned.
