Canyon Ridge Hospital is seeking an Intake Counselor OR Registered Nurse to join our team in Chino, CA. The role focuses on admissions, initial assessments, and referrals for patients across age groups, coordinating with families and medical staff to determine appropriate levels of care.

You will consult with patients and families about current assessments, recommended referrals, and care plans, while supporting a collaborative, diverse clinical environment.

Qualifications

• Master’s degree in social work, psychology, counseling or related field required or an active RN license.
• Minimum of two years experience in a mental health setting preferred.
• Experience with clinical interviewing, patient assessment, and treatment planning.

Canyon Ridge Hospital is a freestanding, 157-bed acute psychiatric facility offering services to adolescents ages 13-17, adults and older adults in the Chino area. We offer a complete spectrum of mental health services for adolescents, adults and seniors.

We are seeking a dynamic and talented Intake Counselor OR Registered Nurse to join our team!

Position Summary

The Assessment & Referral Counselor is responsible for facilitating facility admissions by performing initial assessment and referral activities for all in-coming and potential patients, consulting with potential patients, patient families, medical staff and other hospital staff, and counseling patients and patient families with regard to current assessments, recommended referrals and recommended required levels of care.

Qualifications

• Master’s Degree in social work, psychology, counseling or a related field Required OR a current, active Registered Nurse (RN) license from an accredited nursing program.
• A minimum of two (2) years experience in a mental health setting preferred.
• Working knowledge of the Addiction & Recovery field and experience in clinical interviewing, patient assessment, family motivation, treatment planning, communicating with external review organizations or comparable entities
• Working effectively with people of diverse backgrounds.
