Job summary

Universal Health Services' Fremont Hospital is seeking a part-time Intake Coordinator RN to perform Initial Medical Screening Assessments for incoming patients and coordinate with Medical Staff to direct them to the appropriate level of care. As a member of the Assessment & Referral Team, you will support patient flow, work with colleagues across departments, and participate in CPI training within 90 days.

Qualifications

• Active RN license in California required.
• Associate’s degree in nursing; higher degrees preferred.
• Minimum 1 year direct clinical experience in psychiatric or mental health settings.
• CPR with AED certification current.
• Complete Fremont Hospital CPI training within 90 days of employment.

Responsibilities

• Perform Initial Medical Screening Assessments for incoming patients.
• Coordinate with Medical Staff to direct patients toward an appropriate level of care.
• Collaborate with the Assessment & Referral Team in patient flow.

Education

Qualifications

Education:

• Associate’s degree in nursing required. Bachelor’s Level Degree preferred; Master's in social work, psychology or related mental health field, PhD and PsyD candidates in terminal degree programs with Masters equivalent or a degree from an accredited program in nursing preferred.

Experience:

• Minimum one (1) year direct clinical experience in a psychiatric or mental health setting, with direct experience in patient assessment, family motivation, treatment planning, and communication with external review organizations.

Licensure:

• Currently maintains an active RN license, in good standing, issued by the California Board of Registered Nursing.

Additional Requirements:

• CPR with AED (CPR/AED) certification must be current and presented to Human Resources upon hire.
• You must successfully complete the Fremont Hospital’s non- violent Crisis Prevention Intervention (CPI) training course within 90 days of employment and prior to assisting with preventative intervention measures.
• You may be required to work overtime and flexible hours as needed.
