Sharp HealthCare is seeking a California-licensed LVN to organize and coordinate daily intake/referral processes for our home health agency, ensuring timely initiation of care per physician orders and regulatory guidelines.
The role links hospital case management, medical group physicians, hospice, and home infusion services to coordinate complex patient needs. Weekend rotations and insurance coordination are essential parts of the position.
Shift Start Time: 8:30 AM
Shift End Time: 5 PM
AWS Hours Requirement: 8/40 - 8 Hour Shift

Hourly Pay Range (Minimum - Midpoint - Maximum): $30.370 - $37.950 - $42.510
The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. Organizes and coordinates daily intake/referral processes for the home health agency to ensure that the initiation of care is timely and according to physician orders and within regulatory guidelines. Acts as the key linkage between hospital case management services, medical group physicians and office staff, hospice agencies and home infusion services. Responsible for the coordination of complex patient issues related to home care services.
