Empath Home Health in Sarasota, FL, is seeking a PRN Home Health Intake Coordinator to serve as the primary contact for referrals and patients. You will document referrals, verify coverage criteria, and support the clinical team in scheduling evaluations.

The role requires 2+ years in home health referral management, knowledge of ICD-10, and MS Office proficiency. PRN hours offer flexible scheduling within Empath Health’s mission-driven team.

Qualifications

• Two (2+) years' proven experience in Home Health referral management or a related field.
• Clear understanding of medical terminology and basic ICD-10 coding.
• One (1+) year of supervisory experience or demonstrated ability to manage a team.
• Proficient in Microsoft suite of products, including Outlook, Word, and Excel.

Empath Home Health, a member of Empath Health, is seeking a PRN Home Health Intake Coordinator in Sarasota, FL.

Location: Sarasota, FL; Schedule: PRN.

What You’ll Do

Serves as the primary point of contact for both internal and external customers, providing exceptional customer service.

Receives and meticulously documents patient referrals for home health services, ensuring strict adherence to applicable coverage criteria.

Reviews referrals to determine the appropriate care setting and communicate effectively with the care center, Business Development, and referral sources as needed.

Gathers essential information to assist the clinical team in scheduling evaluation visits.

Communicates pertinent patient information to the care center team through approved processes.

Coordinates with other departments for insurance verification and obtains payer authorization as necessary.

Professionally communicates and follows up with referral sources regarding required documentation for a comprehensive referral.

Meets productivity expectations with quality documentation of complete referrals and all required follow-ups.

Coordinates the evaluation/admission process and may assign field staff to conduct initial assessments.

Ensures compliance with policies, procedures, and regulatory mandates, including adherence to the terms of the Organization's Compliance Program.

Performs other duties as assigned.
