Qualifications

Easterseals MORC is hiring for a Intake Clinician - Hospital Liaison to help make a difference and become part of something bigger than yourself!

This position operates both remotely and in local hospitals in Macomb County, conducting intake assessments, greeting individuals in a subacute setting, and facilitating quick access to intake services while streamlining aftercare for the person served.

Qualifications:

  • Possess a Master’s degree from an accredited college or university with a major in Social Work; AND Have an LMSW from the State of Michigan Department of Consumer and Industry Services (LLMSW Grandfather provision applies); OR
  • Possess a Master’s degree from an accredited college or university in Counseling; AND have a license as a professional counselor (LPC) from the State of Michigan Department of Consumer and Industry Services; AND
  • Must meet qualifications of a Mental Health Professional (MHP) per the Michigan PIHP/CMHSP Provider Qualification per Medicaid Services

Duties and Responsibilities:

  • Completes Screenings, comprehensive Intake Assessments and all intake paperwork.
  • Conducts clinical assessments, develops diagnoses, and participates in developing person/family centered treatment plans with individuals and families.
  • Ensures follow-up appointments are offered/scheduled within required timeframe of intake to coordinate appropriate behavioral health and other needed services.
  • Ensures that appropriate assessments including MichiCANS, LOCUS, and DLA20 are completed at time of intake.
  • Conducts clinical contact to individuals hospitalized according to timeline standards.
  • Manages notifications of hospitalizations, conducts intakes in the hospital setting and conducts greetings and introductions to services in the hospital setting.
  • Responsible for maintaining an electronic and paper chart which includes required documentation of services provided, progress notes, treatment plans, and individual/family rights documents.
