Job summary

Ethan's Crossing in Indianapolis is seeking an Assessment Counselor to join our recovery team and support residents through the admissions process and ongoing care.

You will coordinate clinical care with interdisciplinary teams, drive treatment planning through psychosocial assessments, connect residents with community resources, and manage admissions including benefits verification and authorizations.

Qualifications

• Bachelor's degree (LSW or RN preferred), or Master's degree in social work or counseling with licensure.

Responsibilities

• Coordinate clinical care with interdisciplinary teams and external providers.
• Drive treatment planning by completing psychosocial assessments and building a recovery roadmap.
• Connect the community with warm-referrals to providers for ongoing engagement.
• Manage admissions including verification of benefits and necessary clinical authorizations.
