Job Location: Main Office NJ - Howell Township, NJ
Salary Range: $21.00 - $24.00 Hourly
Description

The Intake and Outreach Specialist will oversee the intake and referral process for individuals eligible for DDD services. The Specialist acts as a liaison between individuals, families, agencies, and state personnel throughout the intake and admissions processes. Under the guidance of the Director of Intake, the specialist handles initial outreach, follow-up, and enrollment through provider relationships and individual engagement. Willingness to travel across NJ and knowledge of social media and digital marketing are required.

Shift: Full Time (40 hours per week)

Schedule: Monday through Friday, 8:00 am – 4:00 pm

Responsibilities:
  • Respond to inquiries about enrollment in services offered at EIHAB.
  • Coordinate referrals, follow-up, and follow-through with referred individuals.
  • Collaborate with program management, families, and Support Coordinators during the intake process.
  • Conduct and schedule meet-and-greets, including dinners and overnight visits as part of admissions.
  • Assist in marketing EIHAB Human Services.
  • Enter new enrollee data into the tracking system.
  • Attend community and staff meetings, as well as mandated trainings.
  • Support networking and outreach, including social media and website management.
  • Assist with agency-wide events to enhance branding.
  • Be flexible to adjust schedules for events, including weekends if necessary.
Qualifications
  • Associate’s degree; Bachelor’s in human services or related field preferred.
  • Bilingual skills are a plus but not required.
  • At least two years of experience with Division of Developmental Disabilities (DDD) services.
  • Minimum one year of work experience with persons with developmental disabilities.
  • Familiarity with NJ benefits, including Social Security and Medicaid.
  • Valid driver’s license meeting agency standards.
  • Goal-oriented, able to work independently and as part of a team in a fast-paced environment.
  • Excellent interpersonal, organizational, verbal, and written communication skills.
  • Proficient in Microsoft Word and Excel; PowerPoint skills preferred.
  • Self-starter, organized, and team player with excellent interpersonal skills.
