Job summary

Hudson Valley Hospice is seeking qualified nurses to join a compassionate interdisciplinary team. You will perform comprehensive assessments and coordinate holistic hospice care, delivering palliative nursing and symptom control in line with physician orders and the IDG plan.

Position requires a NY nursing license, 2+ years nursing experience (hospice preferred), and the ability to document in EMR. Night differential is 5% with full-time and per diem shifts available.

Qualifications

• Graduate of an accredited nursing school.
• New York State nursing license.
• Minimum 2 years nursing experience.
• 1 year ICU/Med-Surg/ER or similar; hospice preferred.
• Venipuncture/IV placement certification preferred.

Responsibilities

• Perform initial and ongoing patient/family assessments to coordinate hospice care.
• Provide palliative nursing care and symptom control per orders and IDG plan.
• Document assessments and interventions in EMR by shift end.
• Develop and prioritize care plan goals with patient/family and IDG.
• Provide anticipatory guidance to help cope with events.
• Communicate effectively in English (verbal and written).
• Revise nursing diagnoses and plan before IDG weekly meeting.

What you will be doing:

• Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural and spiritual assessments to ensure and coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team.
• Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
• Documents and enters into EMR all assessments and interventions before the end of the shift.
• Develop and prioritize the interdisciplinary care plan goals of the patient/family, hospice team and attending physician, prior to team meetings.
• Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur.
• Can effectively communicate in English, both verbally (in person and by phone) and in writing.
• Revises nursing diagnosis, goals and interventions, expected outcomes, and plan of care based on assessment of data and input of patients and family members prior to the weekly IDG meeting.

Work Environment:

• Work in fast paced environment
• Ability to lift at least 30 pounds of supplies and/or equipment
• Moderate/High level of possible exposure to infectious agents.
• Routine duties involve potential for direct exposure to blood or body fluids.

Preferred Education & Experience:

• Graduate of an accredited nursing school.
• Valid New York State nursing license.
• Minimum of at least 2 years of nursing experience required
• Minimum of at least 1 year ICU, Med-Surg, ER or similar required; one year of hospice experience preferred
• Venipuncture and IV placement certification preferred

Hours:

Full Time:

• 12.5 hour shifts (7:00pm-7:30am) - 3 shifts/week
• Every other weekend rotation
• 3 holidays/year

Per Diem

• 12.5 hour shifts (7:0am-7:30pm or 7:00pm-7:30am) both day and night shifts available
• 6 shifts/month with flexibility
• 1 holiday/year

FLSA: Exempt/Non-Exempt

Pay: $87,500-$90,000/yr or $44.87-47/hr; 5% night differential for night shifts
