Qualifications

Responsibilities

• Maintain assigned caseload and provide direct patient care.
• Conduct assessments, individual counseling, and treatment planning.
• Complete group facilitation and documentation in a timely manner.
• Coordinate aftercare and referrals as needed.
• Engage families and participate in MDT/Rounds and trainings.
