Advocate Health Care is seeking a licensed Speech-Language Pathologist to evaluate and treat patients with diverse speech, language, voice, cognitive, and swallowing disorders in a hospital setting.

You will create and implement individualized treatment plans, monitor progress, and coordinate discharges and home programs with families and physicians. Excellent communication and teamwork are essential.

Responsabilidades

• Interprets physician referrals and conducts a speech and language evaluation of patients with various speech, language, voice, cognitive and swallowing disorders.
• Establishes a written plan of care and implements treatment; monitors outcomes and adjusts plans with physician updates.
• Plans for discharge, provides home programs and family teaching; coordinates referrals and equipment as indicated.
• Completes documentation per policy including evaluations, progress notes, and discharge summaries; manages productivity.
• Maintains safety, infection control, and modality knowledge; supports safety initiatives.
• Participates in performance improvement activities and supports student clinical education.
• May supervise non-licensed personnel and provides direction to support staff.
