• Master's Degree in Social Work from an accredited social work program required
• 1 year of experience in an acute or post-acute setting required
• LMSW - Licensed Master Social Work preferred
• ACM - Accredited Case Manager preferred
• CCM - Certified Case Manager preferred

Responsabilidades

• Promote patient and family wellness, improved care outcomes, and transitions planning for patients with complex and psychosocial needs in Inpatient and Emergency Department environments.
• Promote quality of care and effective transitions through collaboration with all service team members, physicians, patients, external agencies and families.
