Job summary

Qualifications

• Master’s Degree in Social Work (MSW) and NY LMSW license.
• At least one year of inpatient Medical/Surgical experience.

Responsibilities

• Coordinate discharge planning and continuum of care.
• Support patients' coping and daily functioning during hospitalization.
• Collaborate with healthcare team to improve patient outcomes.
