Job summary

Loyola University Medical Center (Loyola Medicine), a member of Trinity Health, invites applications for a Social Worker on the Day Shift. The role collaborates with the Care Management Team to deliver coordinated patient care across the continuum, with benefits from day one and competitive shift differentials.

The Social Worker will assess, support, and educate patients and families, coordinate transitions, and contribute to staff training. Requires an MSW and LSW; ER experience preferred.

Qualifications

• Master's degree in Social Work is required.
• LSW licensure required; LCSW preferred.
• 1–2 years of related experience.
• Experience in the ER or acute hospital setting preferred.
• Excellent communication and crisis management skills.

Responsibilities

• Provides psychosocial assessment, intervention, evaluation, and education to patient/families as part of care management.
• Identifies high-risk patients and provides information, support, counseling, and referrals to appropriate resources.
• Serves as liaison for transitions/discharges between hospital and community agencies for the exchange of clinical and referral information.
• Contributes to training of social work professionals.
• Facilitates discharge planning and addresses barriers to a safe discharge.
