Location: Hershey Medical Center, Hershey, PA

Work Type: PRN | FTE: 0.001 | Shift: Day | Hours: 8:00a - 4:30p

Position Summary

Functions as part of an interdisciplinary team to maintain continuity of patient care from admission to discharge, utilizing appropriate criteria for inpatient care and planning timely discharges. Provides emotional support, crisis intervention, and counseling to patients and families, and participates in programs to improve patient access, care quality, and satisfaction.

Minimum Qualifications
  • Masters of Social Work degree from a CSWE-accredited program
Preferred Qualifications
  • At least two years of acute care case management or social work experience
  • Knowledge of PC-based software (Cerner, Navihealth/Curaspan)
  • Current PA social work license
  • Certification in case management from a recognized organization
