Job summary

UHS in Washington state is seeking a Social Worker to support patients' psychosocial needs, facilitate treatment planning, conduct assessments, provide psycho-education, and lead short-term therapy. The role emphasizes evidence-based practice and coordinated discharge planning with community resources.

You will collaborate with clinical teams, maintain compliant documentation, and ensure confidentiality while addressing educational, financial, and care-transition needs for patients and families.

Qualifications

• Must hold AAC or LSWAIC licensure.
• BLS, First Aid, Handle with Care, Verbal De-escalation training provided.
• Preferred: LMHC, LICSW, LMFT, LASW, Licensed Psychologist.

Responsibilities

• Conduct psychosocial assessments to evaluate emotional, behavioral, developmental, social, and support needs.
• Provide individual, family, and group interventions to support treatment goals and patient progress.
• Develop, implement, and update individualized treatment plans with patients, families, and the interdisciplinary team.
• Facilitate treatment planning meetings and coordinate care with physicians, nursing staff, and other disciplines.
• Assist patients and families with identifying resources and addressing psychosocial, educational, financial, and discharge planning needs.
• Monitor patient progress, evaluate treatment effectiveness, and adjust interventions as appropriate.
• Facilitate family meetings and support ongoing communication among patients, families, and the treatment team.
• Maintain timely, accurate, compliant clinical documentation including assessments and discharge planning activities.
• Ensure confidentiality and compliance with regulatory, accreditation, and organizational standards.
