The Role You’ll Contribute
The Social Work Care Manager intervenes with patients with complex psychosocial needs, assists with social program eligibility, funding, community assistance, transitions of care, and discharge planning. They provide crisis intervention and develop discharge plans for high-risk patients, collaborating with the interdisciplinary team to ensure patient-centered care and efficient resource utilization.
The role involves evaluating post-hospital needs, developing transition of care plans, facilitating patient flow, and ensuring safe, smooth transitions. The Care Manager communicates daily with the team, facilitates care across the continuum, removes barriers to care, and educates staff on resource utilization and care coordination. They must be knowledgeable about post-hospital services such as home health, infusion, DME, palliative and hospice care, outpatient services, and community organizations.
The Care Manager adheres to policies, maintains quality care, ensures compliance, and participates in customer service and relationship building.
