Job summary

MedStar Health is seeking a Registered Nurse Case Manager to join the case management team. You will apply clinical nursing expertise to care coordination, discharge planning, and resource utilization from pre-admission through post-discharge.

The role focuses on achieving optimal patient outcomes and cost-effective care across the acute and post-hospital phases. You will collaborate with the interdisciplinary team, monitor progress, and ensure plans address psychosocial and financial needs

Qualifications

• Associate's degree in Nursing (ADN) required.
• Bachelor's degree in Nursing (BSN) preferred.
• 3-4 years clinical experience required and 1-2 years case management experience preferred.
• RN licensure in DC or MD per work location; CCM certification preferred.

Responsibilities

• Collaborate with the multidisciplinary team to develop and coordinate the plan of care.
• Manage a caseload and identify resources needed to implement the plan of care.
• Document and assess patient responses; modify the plan of care as needed.
• Ensure compliance with regulatory requirements and reimbursement mechanisms.
