Job summary

Advocate Health’s Summit site in South Dakota is seeking a proactive Registered Nurse Case Manager for inpatient care. You will coordinate discharge planning, engage patients and families, and support utilization management to ensure safe, efficient transitions.

This on-site role is Mon-Fri 7:30–16:00, 24 hours per week, with weekend rotation and holidays off. It’s a part-time, benefits-eligible position at 0.6 FTE, requiring strong organization and teamwork.

Qualifications

• Licensed Registered Nurse in the state of practice.
• Bachelor’s Degree in Nursing is required.
• Minimum 2 years of clinical nursing experience.

Responsibilities

• Conduct comprehensive assessments and create timely care plans.
• Coordinate discharge planning and care transitions with the care team.
• Collaborate with patients, families, and social work to address barriers to discharge.
