Qualifications

• RN with at least two years of combined experience in Med/Surg, ICU, ED, or similar acute care settings.
• Successful completion of an RN program by date of hire.
• Washington RN license (or compact license) within 6 months of hire.
• Basic Life Support certification within 3 months of hire.

Responsibilities

• Coordinate post-discharge care to assure timely, safe hospital discharges.
• Complete and document patient assessments with age-appropriate psychosocial support.
• Develop mutually agreed care plans with patients and families.
• Facilitate safe transitions and resolve barriers to transfers.
• Maintain contact with interdisciplinary staff to support transitions.
• Ensure regulatory standards are met in collaboration with the health care team.
• Perform duties as requested.
