Perform assessments, develop and update individualized care plans, administer medications and treatments, monitor patient progress, and collaborate with physicians, therapists, and case managers. Engage in patient and family education, document in EMR, participate in interdisciplinary team conferences, and support a patient-centered, faith-based culture emphasizing safety, respect, and clinical excellence.
