Equiliem in Bradenton, FL is seeking a dedicated Physical Therapist to join our inpatient rehabilitation facility and acute care team. You will evaluate patients, develop personalized treatment plans, and collaborate closely with physicians, nurses, therapists, patients, and families to restore mobility and function.

Ideal candidates have a state license, IRF experience, and strong communication and teamwork skills.

Qualifications

• Current Physical Therapist license in the applicable state.
• Graduate of an accredited Physical Therapy program.
• Willingness to work on the Inpatient Rehabilitation (IRF) unit.
• Strong critical thinking and clinical reasoning skills.
• Excellent communication and teamwork abilities.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop and implement individualized treatment plans.
• Provide evidence-based therapeutic interventions to restore function and mobility.
• Complete patient reassessments and discharge planning.
• Participate in interdisciplinary team conferences.
• Provide patient and family education and training.
• Recommend and train patients in the use of adaptive devices, assistive technology, and mobility equipment.
• Accurately document evaluations, treatments, and patient progress in Meditech.
• Collaborate with rehabilitation, nursing, and medical teams to optimize patient outcomes.
