Job summary

Responsibilities

• Perform initial examination and evaluation to establish impairment, prognosis, and plan of care.
• Provide or supervise therapy interventions aligned with evaluation and plan.
• Involve patient and care team in planning and reassessment.
• Plan discharge in consultation with team and ensure follow-up.
• Document all patient management activities and communicate results with the care team.
• Supervise rehab aides and office staff.
• Manage schedule to accommodate census and caseload; report issues.
• Attend team meetings and communicate patient status.
• Pursue ongoing professional development.
