Job summary

Legent Health in Carrollton, TX is seeking a Registered Nurse for the Rehab Unit on a full-time days schedule. The role focuses on delivering direct/indirect patient care to adult rehab patients, coordinating with an interdisciplinary team, and developing comprehensive nursing plans of care.

Strong assessment, documentation, and educator roles are essential. Qualifications include Texas RN licensure, BSN preferred, and current BLS/CPR certification; CRRN certification is preferred.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current licensure to practice professional nursing in Texas.
• Current American Heart Association (AHA) Healthcare Provider CPR (BLS).
• Certification in specialty preferred; ARU within 2 years of hire.
• ARU experience preferred (1 year medical-surgical and 1 year home health)

Responsibilities

• Assess/reassess patient condition based on observation, diagnosis, data, and family input.
• Develop/revise nursing plans of care with expected outcomes.
• Collaborate with rehab team to implement treatment plans addressing medical and functional deficits.
• Deliver safe, organized rehabilitation nursing interventions.
• Attend weekly team conference to report patient status and progress.
• Document patient care and complete CARE scoring for CMS indicators.
• Provide patient and family education regarding treatments and post-hospital care.
• Assist in orientation and training of new staff and serve as preceptor when requested.
