Qualifikationen

• Graduate of an accredited NP program.
• Current NP certification and state authorization to practice.
• Clinical experience managing medically complex patients.
• Strong communication and teamwork with an interdisciplinary team.

• Perform patient evaluations and follow-up visits within your scope.
• Assess and manage acute and chronic conditions.
• Review labs and imaging; coordinate follow-up.
• Communicate with physicians, rehabilitation clinicians, specialists, patients, and families.
• Support treatment planning and transitions of care.
• Complete accurate documentation in EMR.
