HCA Florida Largo West Hospital in Largo, FL is hiring a Clinical Rehab Specialist to act as a liaison between case management, physicians and referral sources. You will screen and place patients, coordinate admissions, and support ongoing education for staff and families.

Qualified candidates have a clinical license (RN/OT/PT/SLP), at least 2 years of in-clinic experience, and BLS. The role emphasizes timely communication and collaboration across multidisciplinary teams.

Qualifications

• Graduate of an accredited school in clinical specialty.
• Minimum 2 yrs recent clinical care experience.
• Licensed professional in a clinical health care specialty (RN, OT, PT, or SLP).
• Pass FIM within 90 days of hire and maintain certification.
• RN CRRN preferred within 2 years of accepting position.
• BLS certification (AHA) required.
• Clinical experience in inpatient rehab admissions coordination.

Responsibilities

• Coordinate intake and pre-admission screening process and document criteria.
• Conduct on-site and off-site clinical evaluations for rehab suitability.
• Advise referral sources on inpatient rehab criteria and follow up.
• Serve as primary contact for case management and referring physicians.
• Develop call-plan to optimize time with referral sources and education.
• Provide face-to-face or in-service education to staff and patients.
