Responsibilities

• Create plan of care using assessment, diagnoses, intervention and evaluation of outcomes.
• Identify patient safety concerns and apply appropriate resources.
• Provide education based on patient learning needs and cultural considerations.
• Document care plans, assessments and handoffs for seamless team communication.
