Job summary

Horizon Health seeks a Social Worker (MSW/LMSW) to join Abbeville General Hospital's 16-bed inpatient psychiatric unit serving adults 18+. The role involves psychotherapy services, assessments, treatment planning, group/individual/family therapy, and discharge planning within a multidisciplinary team.

Qualifications include a Master’s in social work, a valid state license, and CPR/BLS/ACLS certifications with crisis prevention experience.

Qualifications

Master's degree in social work required, LMSW preferred.
Valid state social work license.
CPR, BLS, and ACLS certifications required.
Crisis prevention experience

Responsibilities

Implements a treatment plan of care for patients based on a thorough assessment of the patient’s symptom presentation and input from all data sources.
Engages in timely discharge planning in coordination with patient family, nursing staff and attending physician.
Participates as an active member of the interdisciplinary and treatment teams; attends Unit and Hospital meetings.
Conducts individual, group and education psychotherapy sessions as needed, including weekends.
Performs other duties as assigned by this position
