Job Description
Part Time, 40 hours every 2 weeks
Schedule: 2 weekdays including Fridays and an alternating weekend shift.
The Case Therapist I embodies the mission, vision, and values of NM, adheres to the Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and standards.
This role involves assessing patients, treatment planning, and providing direct services such as individual, group, and family counseling. The therapist coordinates care by communicating with the treatment team, patients, and families, documenting in medical records, and discharge planning.

Responsibilities:

  • Maintain a friendly, approachable, professional demeanor and appearance.
  • Provide accurate information and timely updates to patients and customers.
  • Address questions and concerns promptly or direct them to appropriate resources.
  • Assist patients and customers in finding their way within the facility.
  • Use effective service recovery skills to resolve issues or service breakdowns.
  • Demonstrate teamwork by assisting co-workers across departments.
  • Communicate effectively, respecting diverse opinions and contributions.
  • Use organizational resources efficiently.
  • Manage work schedules to complete tasks on time.
  • Participate in continuous improvement efforts.
  • Contribute to cost reduction, process streamlining, and service growth.
  • Document accurately and promptly in the medical record using Microsoft Word or other automated systems.
  • Retrieve patient information to enhance case knowledge and facilitate care transitions.
  • Conduct individual and group counseling effectively, and provide family counseling as assigned to support family systems.
  • Clearly communicate information about self-help groups to support patient transition and ongoing support.
  • Understand DSM diagnoses and develop therapeutic interventions based on clinical contact and input from the treatment team.
  • Establish rapport with patients and support systems to encourage trust, disclosure, and behavioral change for recovery.

Qualifications

Required:

  • Experience working with substance abuse and mental health populations
  • IL LPC or LSW licensure
  • Master's degree in social work, psychology, counseling, or related field
  • Experience and training in assessment, individual, family, and group counseling
  • Proficiency with computerized medical records systems

Preferred:

  • 2-5 years in intensive treatment or mental health settings
  • IL LIC CPC, LIC LCSW, LIC MSW
  • CERT CADC
  • Presentation skills including lectures and didactic presentations
