Job summary

NorthBay Health is seeking a Population Health R N Inpatient Case Manager (PHRNICM) to deliver complex case management for high‑risk populations and to assist within the inpatient case management department. The role emphasizes care coordination and collaboration with the care team to improve outcomes.

The position requires a BSN, California RN license, CCM/ACM within two years, and at least two years of clinical experience including one year in case management.

Qualifications

• Graduated from an accredited nursing program; BSN required.
• CCM or ACM certification within two years of hire.
• Minimum of two years of relevant clinical experience.
• Minimum one year of case management experience is required.

Responsibilities

• Identify high‑risk patients for resource utilization from referrals and EHR data.
• Assess patient needs and develop individualized care plans with objectives and barriers.
• Coordinate care across disciplines to ensure timely delivery of appropriate services.
• Follow up with patients and providers to address health care needs and resources.
• Provide health education and empower patients and families to make informed choices.
