Job summary

Banner Health is seeking a Physical Therapist to join a large multidisciplinary team to evaluate and treat inpatient therapy needs. The position supports Banner Fort Collins Medical Center and offers Registry/Per Diem opportunities within Banner Health.

Qualifications

• Graduated from an APTA accredited school of Physical Therapy.
• Current Physical Therapy license in the state of practice.
• BLS certification required.
• Ability to become proficient with computer software in automated patient care environments.
• Arizona Fingerprint Clearance Card required for Banner Home Care/Hospice or Boswell Skilled Nursing Facility.

Responsibilities

• Evaluate patient functional abilities and develop a plan of care.
• Provide and supervise PT treatment in accordance with physician referral and standards.
• Document treatment and progress per guidelines.
• Communicate results with physicians, nurses, patient and family.
• Supervise PTA and therapy support staff as assigned.
