Job summary

Qualifications

• At least 1200 hours of inpatient pharmacy experience within the last 5 years.
• Doctorate of Pharmacy (Pharm.D.) or equivalent degree required for licensure.
• California Pharmacist License required at hire; NPI required at hire.
• ACLS and BLS within 3 months of hire; American Heart Association.
• Experience with aseptic technique and sterile IV preparation.

Responsibilities

• Monitor and optimize patient medication therapy in collaboration with the health care team.
• Adjust doses, orders labs, and administer medications under protocol.
• Review medication orders for appropriateness and formulary adherence.
• Advise patients and caregivers on medication use, storage, and safety.
• Document accurately in medical records and ensure medication safety.
