Centra Health in Lynchburg, Virginia, is seeking a compassionate Nurse Practitioner or Physician Assistant for its Palliative Care team. This role involves providing holistic care for patients with advanced illnesses, working alongside a multidisciplinary group to enhance quality of life.

Qualifications

Job Description

We are seeking an exceptional and compassionate Nurse Practitioner or Physician Assistant to join our dedicated Palliative Care team. This is a meaningful opportunity to make a lasting difference in the lives of patients and families during some of their most vulnerable moments.

Responsibilities

• Provide holistic and supportive care for patients with advanced illnesses, primarily in oncology, neurology (stroke, dementia, etc.), pulmonary (COPD, respiratory failure, etc.), and gastrointestinal diseases.


• Collaborate with a multidisciplinary team focused on improving quality of life and managing symptoms.

You Can Expect

• 8:00 – 5:00pm Monday - Friday


• 6 – 10 patients per day, mix of new admits and follow-up patients


• 2 Physicians and 4 APPs


• Collaborate with families, patients, social workers, and care teams across the system


• Call – 1:4 one weekend a month and one weekday a week; calls are usually taken from home but may require occasional in-person visits


• Robust 2-3 month orientation process

Qualifications
