Role Overview

We are seeking an exceptional and compassionate Nurse Practitioner or Physician Assistant to join our dedicated Palliative Care team. This role offers the chance to make a lasting difference in the lives of patients and families during some of their most vulnerable moments.

About the Team

Centra Medical Group (CMG) is a network of nearly 600 employed providers serving a community of over 600,000 across Central and Southern Virginia. Our footprint spans Lynchburg, Danville, Farmville, Moneta, and Bedford, offering a wide range of primary and specialty care services. CMG supports the Lynchburg Family Medicine Residency and partners with academic institutions such as UVA, VCU, VCOM, and Liberty University.

Work Hours and Schedule

• 8:00 – 5:00 pm Monday‑Friday
• 6 – 10 patients per day (mix of new admits and follow‑up patients)
• Team: 2 Physicians and 4 APPs
• Collaboration with families, patients, social workers, and care teams across the system
• On‑call: one weekend a month and one weekday a week (usually from home; occasional in‑person visits required)
• Robust 2‑3 month orientation process

Responsibilities

Provide holistic and supportive care for patients with advanced illnesses—primarily in oncology, neurology (stroke, dementia, etc.), pulmonary (COPD, respiratory failure, etc.), and gastrointestinal diseases. Work within a collaborative interdisciplinary team focused on improving quality of life and managing symptoms.

Required Qualifications

• Graduate of an accredited nursing program with a Master’s of Science in Nursing
• Minimum of two (2+) years of progressive experience in palliative care or a related specialization
• Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority; holds active RN license in Virginia or multistate RN privilege
• Nationally recognized certification in field of specialization
• Current BLS and ACLS certifications
• Experience with disease processes and symptom management
• Strong communication skills, empathy, and experience with end‑of‑life care and advanced care planning
