NJ Palliative Care Solutions is hiring a Nurse Practitioner or Physician Assistant to provide hospital-based palliative care at Cooperman Barnabas Medical Center in Livingston, NJ. Palliative care experience is required for this inpatient role. 

Active New Jersey licensure and national board certification are required. The position emphasizes structured mentorship, physician/APP backup, and a collaborative interdisciplinary team with Monday–Friday scheduling and a strong rewards package.

Qualifikationen

• Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.
• DEA + CDS registration, or ability to obtain by start date.
• 2+ years of palliative care experience required.
• Palliative care certification (e.g., ACHPN / CHPN) preferred.

Aufgaben

• Provide hospital-based palliative care consultations and follow-up visits across the hospital continuum.
• Manage complex symptoms for patients with advanced illness.
• Lead goals-of-care and advance care planning conversations with patients and families.
• Coordinate with partner SNFs and hospices when appropriate.
• Integrate into hospital workflows and contribute to palliative medicine practice.
• Complete timely documentation aligned with clinical, billing, and quality standards.
• Spend time on discharge planning and transitions from inpatient to next setting.
