UNC Health in Hillsborough, NC seeks a Social Worker - MSW (Inpatient) to support patients and families throughout the care journey. You will conduct assessments, develop patient-centered care plans, facilitate resources, and advocate for timely services.

You will collaborate with an interdisciplinary team, monitor progress, and coordinate care across providers while communicating with payers when needed. This role emphasizes empathy, cultural sensitivity, and confidentiality.

Formación

• Master's Degree in Social Work
• Two (2) years health care experience strongly preferred
• Knowledge of mental health conditions, diagnostic criteria, and evidence-based treatment modalities
• Strong assessment and diagnostic skills
• Excellent communication and interpersonal skills
• Ability to collaborate effectively as part of an interdisciplinary team
• Empathy, compassion, and cultural sensitivity
• Strong organizational and time management skills
• Ability to maintain confidentiality and adhere to ethical guidelines

Responsabilidades

• Assess patients' mental, emotional, and social needs to identify barriers and plan care.
• Collaborate with providers, patients, families and team on individualized care plans; facilitate resources and referrals.
• Monitor and evaluate progress; ensure appropriate care progression and continuity.
• Advocate for patients within the healthcare system and coordinate communications with payers when needed.
• Engage in active listening to understand concerns and provide empathetic support.
• Respect diverse backgrounds; tailor interventions to cultural and personal contexts.
• Build trust-based relationships through confidentiality and professionalism.
• Identify risk factors and prioritize interventions based on needs.
• Carry out delegated tasks and interventions as part of the care team.
• Coordinate referrals, follow-ups, and care across providers and settings.
• Communicate with payers for authorization and documentation requirements
