Job summary

• National Military Courtesy License upon hire.

Responsibilities

• Provide professional, patient-centric care management in an acute care environment.
• Perform assessment, planning, care coordination, discharge planning and documentation of interventions.
• Advocate for patients and identify resources to address social, financial, cognitive/behavioral or legal barriers to access.
