Job summary

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• 1 year of Social Work experience in a healthcare setting.
• 1 year of acute care experience or completion of TIPs/Case Management Orientation.

Responsibilities

• Provide professional, comprehensive patient-centric care management services for at-risk patients in an acute care environment.
• Conduct assessment and planning, coordinate care, and discharge planning.
• Document interventions, regulatory compliance, and patient advocacy.
• Identify resources to address social, financial, cognitive/behavioral or legal barriers to care access.
