Job summary

Providence St. Jude Medical Center in Fullerton, CA is seeking a Care Manager MSW to join our inpatient team.

This full-time role requires 8-hour evening shifts and focuses on patient-centered care management for at-risk patients in an acute care setting. The MSW Care Manager will assess and plan, coordinate care, discharge planning, document interventions, and advocate for patients while ensuring regulatory compliance.

Qualifications

• Master's degree in Social Work from an accredited CSWE program.
• At least 1 year of Social Work experience in a healthcare setting.
• At least 1 year experience in acute care or completion of TIPs or Case Management Orientation.

Responsibilities

• Assess and plan care for at-risk patients in an acute care setting.
• Coordinate care and discharge planning for patients.
• Document interventions and ensure regulatory compliance.
• Advocate for patients to address social, financial, cognitive/behavioral, or legal barriers to access.
